What Is Aetna?
Aetna is a national health insurance company. On its own website, Aetna describes itself as a proud CVS Health company and notes an expansive network of more than 1.5 million care professionals. You may have Aetna through your job, through a Medicare plan, or through another type of coverage.
Aetna publishes its own behavioral health rules for its plans, which means that for many members, Aetna itself reviews requests for substance use disorder care. Some employers use a separate company for behavioral health, though. If the back of your ID card lists a different name or number for mental health and substance use, contact that company about rehab benefits.
Does Aetna Cover Rehab? What Plans Typically Include
Federal law sets a baseline. According to HealthCare.gov, all Marketplace plans cover mental health and substance abuse services as essential health benefits, and plans can’t deny you coverage or charge you more because of a pre-existing substance use disorder. For job-based plans that offer these benefits, the Mental Health Parity and Addiction Equity Act says copays, deductibles and visit limits for mental health and substance use care can’t be more restrictive than those for medical and surgical care.
Within those rules, Aetna plans commonly include several levels of addiction care:
- Medical detox in a hospital or other supervised setting
- Inpatient and residential treatment for people who need 24-hour care
- Partial hospitalization (PHP), a full-day outpatient program
- Intensive outpatient (IOP), several hours of treatment a few days a week
- Outpatient counseling and psychiatric care
Aetna also runs a network called the Aetna Institutes of Quality behavioral health network, which recognizes providers that meet certain quality and cost-efficiency standards, including facilities that treat substance use disorders.
What about medication for addiction?
Medications can be an important part of recovery for some people. SAMHSA notes that buprenorphine should be prescribed as part of a comprehensive treatment plan that includes counseling and other services. Coverage for these medications usually falls under your pharmacy benefit, so ask Aetna what your plan covers and what you would pay.
Does Aetna Require Precertification for Rehab?
Precertification (also called prior authorization) means Aetna reviews and approves certain services before you get them. Aetna’s behavioral health precertification list, effective August 1, 2024, says precertification is required for:
- Inpatient stays, including substance use disorder admissions
- Residential treatment
- Partial hospitalization programs
- Out-of-network care you want covered at the in-network benefit level
Intensive outpatient treatment is not on that list. The list also notes that it does not apply to some plans, such as Traditional Choice and indemnity plans, so your plan documents are the final word.
When Aetna reviews addiction treatment, it uses clinical guidelines. Aetna’s parity disclosure form states that for substance use disorder treatment, Aetna utilizes criteria developed by the American Society of Addiction Medicine, known as the ASAM Criteria. These guidelines look at things like withdrawal risk, other health conditions, readiness to change and your home environment to match you with the right level of care. Your treatment team typically handles this review for you.
In-Network vs. Out-of-Network Aetna Coverage
Staying in network usually keeps your costs lowest. Depending on your plan type, out-of-network care may cost you more or may not be covered at all, and Aetna requires precertification if you want out-of-network care covered at the in-network level. Using Aetna for Outpatient Rehab at The Ranch Pennsylvania Outpatient Services
We offer two main levels of outpatient addiction care in Lancaster, PA:
- Partial hospitalization program (PHP): full-day programming five days a week at our Outpatient Center. Most clients complete PHP in about 8 to 12 weeks. Under Aetna’s list, PHP generally needs precertification.
- Intensive outpatient program (IOP): three-hour group sessions three days a week while you live at home, typically for about 5 to 6 weeks.
PHP clients can also stay at our Outpatient Residence, a 16-bed, substance-free home in Lancaster with 24/7 support staff and transportation to programming. Care includes CBT, DBT, motivational interviewing, family therapy, psychiatric care and medication management, with medication-assisted treatment available when clinically appropriate.
