Symptoms of depression
A diagnosis of major depression requires symptoms most of the day, nearly every day, for at least two weeks, including at least one of the first two below:
- Persistent sad, empty or hopeless mood
- Loss of interest or pleasure in activities you used to enjoy
- Significant changes in appetite or weight
- Sleeping too much or too little
- Fatigue or loss of energy
- Feelings of worthlessness or excessive guilt
- Difficulty thinking, concentrating or making decisions
- Restlessness or feeling slowed down
- Thoughts of death or suicide
Depression can also appear as irritability, anger, physical pain, or withdrawal from friends and family. Some people function at work while feeling nothing inside; others cannot get out of bed. What causes depression?
Depression arises from a combination of genetics, brain chemistry, hormonal changes, medical conditions, trauma and life stress. Grief, job loss, relationship problems, chronic illness and isolation can all trigger an episode, especially in people with a family history. Substance use is both a cause and a consequence: alcohol is a depressant, opioid and stimulant withdrawal produce profound low mood, and people who are depressed often use substances to feel something or to feel nothing.

About a third of people with major depression also have a substance use disorder. Treating one without the other rarely works. Our programs are built for both: clients with co-occurring conditions receive integrated care from one team, and clients without substance use receive dedicated mental health treatment.

Levels of care
- Inpatient or residential psychiatric care – for people who are in crisis or need 24-hour supervision to stay safe.
- Partial hospitalization program (PHP) – full-day psychiatric and therapeutic care five days a week at our Lancaster Outpatient Center, with supportive housing available. Most clients complete PHP in 8–12 weeks.
- Intensive outpatient program (IOP) – three-hour group sessions three days a week while you live at home, typically for 5–6 weeks.
- Outpatient therapy and medication management – ongoing weekly care after you complete a structured program.
How we treat depression
Treatment at The Ranch Pennsylvania Outpatient Services combines psychiatric care with therapy. Our psychiatric providers evaluate every client and manage medication, including antidepressants and treatment for co-occurring conditions. Cognitive behavioral therapy helps clients recognize and change the thought patterns that sustain depression; behavioral activation rebuilds routine, activity and connection; dialectical behavior therapy addresses emotional intensity; and group therapy reduces the isolation that depression thrives on. Clients who have experienced trauma can access cognitive processing therapy.
