Does Cigna cover rehab? In most cases, yes. How Cigna Is Organized Today

Cigna Healthcare is the health insurance business of The Cigna Group, which also includes a sister company, Evernorth Health Services. For people with Cigna coverage through an employer, Cigna notes that behavioral health benefits are administered by Evernorth Behavioral Health. That means Evernorth may be the company that reviews addiction treatment requests, even though your ID card says Cigna.

One recent change is worth knowing. In March 2025, The Cigna Group completed the sale of its Medicare Advantage and Medicare Part D businesses to Health Care Service Corporation (HCSC). If you had a Cigna Medicare Advantage plan in the past, your plan is now run by a different company, so check the name on your current card and member materials.

Does Cigna Cover Rehab for Drug and Alcohol Addiction?

Cigna’s employer plan materials list substance use benefits that include inpatient and outpatient treatment services, combined with follow-up case management, along with coaching and support programs. Cigna also says there is no separate deductible for mental health and substance use benefits, though details vary by employer plan.

Federal law supports this coverage. Under the Affordable Care Act, substance use disorder treatment is an essential health benefit that Marketplace plans must cover, and plans can’t deny you coverage because of a pre-existing condition, including a substance use disorder. The Mental Health Parity and Addiction Equity Act adds that copays, deductibles and visit limits for addiction care generally can’t be stricter than those for medical or surgical care.

Levels of Addiction Care Cigna May Cover

Cigna’s own guide to treatment for substance use disorders describes a range of care options, starting with a full evaluation. Levels that Cigna plans commonly include are:

  • Inpatient detox: for people who need 24-hour medical care while withdrawing. Alcohol and benzodiazepine withdrawal can be life-threatening and should always be medically supervised.   • Residential treatment: live-in care for people who need more structure.
  • Partial hospitalization (PHP): Cigna describes this as treatment five to seven days a week for six to eight hours a day. Our addiction partial hospitalization program offers full-day programming five days a week.
  • Intensive outpatient (IOP): several visits a week of about three to four hours each. Our addiction intensive outpatient program meets three days a week for three-hour sessions.
  • Routine outpatient care: individual counseling with a therapist, sometimes with a psychiatrist or addiction specialist.

If you may benefit from medication-assisted treatment, ask your plan how those medications are covered. Drug coverage usually falls under your pharmacy benefit, and rules differ from plan to plan.

Prior Authorization and Medical Necessity

Cigna says some treatments may require prior approval before you get the service. For higher levels of addiction care, a clinical team reviews whether the program fits your needs. For adults, Evernorth says it uses The ASAM Criteria, 4th Edition, for substance use level-of-care reviews (the 3rd Edition for customers 17 and under), unless a law or contract requires something else. In plain terms, the reviewer looks at your whole situation to match you with the right level of care.

There has also been good news for outpatient care. According to The Cigna Group, Evernorth Behavioral Health removed prior authorization for routine outpatient mental health and substance use care and for intensive outpatient programs. PHP, residential and inpatient care may still need approval. Your provider usually handles these requests, but it helps to know they exist.

In-Network vs. Out-of-Network Rehab

When people ask “does Cigna cover rehab,” the network question often makes the biggest difference in cost. Using an in-network provider usually means lower costs and fewer billing surprises.
